FRONTLINE SERVICE  :   Out of Town Registration and Reporting


Out-of-Town Registration / Reporting applies to persons whose Vital events (birth, death, marriage) took place outside the  Municipality but are not indicated in the Registry Books of that Place and in the records of NSO.


1.Negative Certification from the NSO
2.Negative Certification from the Office of the Municipal   
Civil Registrar where the event took place
3.Affidavit of two (2) disinterested persons
4.Postal Money Order (minimum of Php 300.00)

1.Baptismal Certificate/Certificate of Dedication
2.Voter’s Registration Record
3.School Record
4.Any valid identification card

1.Transcription from the hospital records/report of the
attending physician
2.Client’s original copy of the death certificate (if    

1.Client’s original copy of the marriage certificate/contract
(if available)
2.Transcription from the church/court records where the      
event was solemnized

Fees to be Paid  : Php 300.00     Postal money order

Service Schedule : Monday to Friday ( 8:00 am to 12:00 hn, 01:00 pm to 05:00 pm)
